Objectives: The objective was to assess the impact of perceptional changes in diabetic patients through a Medication Therapy Management program in South Kerala, India. This study was designed exclusively to identify the pharmacist’s role in patient care using the pharmacist-led MTM program. Methods: A 15-item questionnaire was designed to assess the knowledge, attitude, and practice of diabetic patients. The KAP questionnaire consisted of 7 knowledge questions, 3 attitude questions, and 5 practice questions. A total of 104 diabetic patients from general medicine were selected for the study over a 6-month duration. The questionnaire was administered to the patients before and after providing MTM services. Key findings: The study revealed that the knowledge and attitude of diabetic patients improved after receiving MTM services, whereas practice did not improve, supporting previous study evidence. Conclusion: The study identified that the pharmacist-led MTM program has the potential to improve optimal diabetes management by involving patients as part of the healthcare team. The ability of the clinical pharmacist to build trusting relationships with patients and providers is essential for the success of such a program.
